To ensure you receive the benefits for which your level of sponsorship qualifies, please return this form along with your payment. If you cannot make a full payment, please contact us to make alternate payment arrangements. Once your sponsorship is received you will be mailed a thank you letter with our tax identification number within 2 to 3 business days after full payment is received (checks may take 7 to 10 days).
